To a member of the public, I can see two emoticons arising:

First, alarm 😡 at the incidence of errors in hospitals;

The second, puzzlement 😮 at the quoted solution, Ten Thousand Feet.

“What’s this?” I can hear members of the public ask?

“Is this even a ‘fix’?”

But if you knew the problems around implanting a safety culture into healthcare, you would see that this is the best fix there is.
